Shy 38 Inc. Summer Camp Registration

2. Permission Form
   

I, the undersigned, hereby grant permission for my child to participate in all camp activities hosted by Shy 38 Inc. located at 2100 Wild Horse Rd Lawrence, KS 66044 Activities may include, but are not limited to: animal care education, light outdoor chores, educational crafts, games, and interaction with farm animals under supervision.

I understand that appropriate safety measures will be taken at all times and that my child will be under supervision. I give permission for emergency medical care to be administered in the event I cannot be reached.

I give permission for my child to be photographed for use in Shy 38 Inc.'s educational and promotional materials.

3. Liability Waiver

I understand and acknowledge that participation in the Shy 38 Inc. summer camp may include interactions with animals and outdoor activities that carry inherent risks, including the risk of injury.

I hereby release and hold harmless Shy 38 Inc. and staff, volunteers and representatives from any and all liability claims, demands and causes of action arising out of or related to any loss, damage or injury that may be sustained by my child or to any property belonging to me or my child, while participating in activities at or sponsored by Shy 38 Inc.

I understand that it is my responsibility to inform camp representatives of any physical or mental conditions that may affect my child's ability to participate in the program.
